1. Specify if the order is for pickup or delivery, with these options meaning:
a) Pickup your book at 4130 W. Jefferson Blvd. Suite I-3, in Fort Wayne, IN 46805
b) Delivery - include a physical address other than the one listed above, like your home or the home of someone lucky enough to receive the book(s)
2. Book title for all books being ordered
3. Author of the book(s)
4. Edition: paperback, hardback, or other specifications if applicable such as large-text, audio, etc.
